Job Responsibilities: Teaching individuals to assure the acquisition of independent living skills, provide supervision, and provide for each person’s care needs. Work effectively with individuals, families, other staff and support services staff. Communicate, orally and in writing. Follow all licensing regulatory standards to include health and fire regulations. Participate in the development of the Individual Program Plan (IPP) to determine services needed to achieve support plan outcomes. Follow a schedule of services to be provided including the days of the week and the time of day. Maintain service log for each date service is provided. Complete monthly progress reports noting progress toward achieving the support plan outcomes. Report any incidents of abuse/neglect in accordance with the Disabilities Act, to the authorities (Abuse Registry). Ensures a family home atmosphere with routines, which promotes interaction and allows for risk taking, while fostering growth and independence is maintained in the home. Responsible for the completion and implementation of all direct care, behavioral training, programming, and physical/nutritional management programs/routines for the people living in the home. Provides supervision, instruction, and assistance in the completion of daily personal hygiene and grooming, household maintenance, personal routines, social skills, and activities of daily living for individuals residing in the home. Implements training procedures/behavior programs to meet outcomes listed on the support plan and proper documentation of skill acquisition and behavioral programs. Facilitate leisure/recreational activities; participate in the planning and actual involvement in outside recreational activities. Complete documentation of each person’s activities as required and the maintenance of all records including daily logs and individual monthly assessment process following safety guidelines. Transport people to day program, outside appointments, church, and other community events as assigned. Monitor each person’s health status and report concerns to the Group Home Manager. Carryout emergency medical related actions as trained. Assist in the provision of health care needs, including preparation, administering and charting medication. Maintain home atmosphere. Model personal and social skills, defusing conflicts and reinforcing behavior. Implement behavior programs and skill acquisition training activities. Complete various household cleaning/maintenance tasks as assigned to maintain the home. Assist in conjunction with other home staff, in the responsibility for the overall operation of the home to include such tasks as meal preparation, shopping, transportation, housecleaning, laundry, yard maintenance, etc. React immediately and in accordance with emergency procedures in the event of a fire drill or actual fire. Assist other personnel when requested. Work in accordance with the devised work schedule. Participate in all staff meetings and training sessions. Ensure the health, safety and well being of each individual. Cooperate fully with any other individuals involved with the person’s care including the family, the Support Coordinator, other staff, etc. Report any behavior, incidents, etc. (ex. Seizures) to the administrative office immediately and complete an unusual incident report form. Observe rules prohibiting smoking, eating, or drinking in the individual’s home, on vans or in the building in any other than designated areas. Participate in training to improve training and evaluation skills. Maintain current CPR, 1t Aid, and ACT Certification. Any other assigned duties.

Job Title: Lawn Crew Trainer/Supervisor

Maintain current CPR, 1st Aid, and ACT Certification. Valid Florida Drivers License, point-free driving record, and non-school bus driver certification required.

Job Responsibilities: Functions in community settings providing supports and training as specified in the Individual Program Plan. This position is assigned as a supervisor for the lawn maintenance crew. Complete work crewmember assessments and evaluations. Participate in the development of the Individual Program Plan (IPP). Develop and implement specific training procedures. Collect and record all data of the training procedures. Report progress (or lack of) to the Program Coordinator. Complete monthly progress reports. Maintain safety within the station and following all safety procedures. Maintain quality control in all phases of work. Account for all work assigned to the workstation. Complete and submit daily work production reports and worker daily time sheets. Maintain utility trailer and vehicle. Responsible for completing lawn work as described by the lawn work order and customer. Supervise all members of the crew while working on assigned jobs. Complete the lawn maintenance work production daily. Issue bills for services to lawn customers. Supervise the use and care of lawn maintenance equipment and reports any problems with the equipment. Supervise the loading and unloading of the lawn maintenance utility trailer. Reports any behavior, incidents, etc. (ex. Seizures) to the administrative office immediately and completes an incident report form. Supervise medication for any work crewmember on the work crew who takes prescription medication during the workday. Supervise work crewmembers continually. Substitutes for any other trainer as requested. Observe starting and ending time and break and lunch time to set an example for workers. Observe rules prohibiting smoking, eating, or drinking on the vans or in the building. Participate in training to improve training and evaluation skills. Any other assigned duties.

Work Schedule: Monday – Friday 9am – 5pm.

Job Title: Training Specialist II

Requirements: High School Diploma or equivalent. 3 years of experience working with developmentally disabled persons or related experience. Industrial or Production work experience preferred. Valid Florida Drivers License, point free driving record, and non-school bus driver certification required. Must have computer skills. Employer conducts drug screening, background reference and motor vehicle record checks.

Job Responsibilities: Be the lead trainer in the adult day training program. Responsible for the supervision of both the implementation and in the development of the Individual Program Plans (IPP) and the meeting contract work requirements. Complete assessments and evaluations of persons served. Participate in the development of the specific training programs. Develop and implement specific training procedures. Collect and record all data of the training procedures. Report progress (or lack of) to the program coordinator. Complete monthly progress reports. Assure that Policies and Procedures are being followed in the production department. Orient persons served in job skills and job habits and instruct in production/work procedures. Assure that wage & Hour regulations are being met. Set up the work area for productivity. Develop and implement procedures to assure the meeting of production quotas. Supervise receipt and delivery of sub-contract items. Maintain safety within the workstation and following all safety procedures. Maintain quality control in all phases of work. Complete and submit daily work production reports and worker daily time sheets. Maintain workroom. Supervise all members of the crew working on assigned jobs and arrange for supervision if personally unable to directly supervise. Supervise the use and care of equipment and report any problems with the equipment. Provide personal care assistance to individuals such as feeding meals and snacks, changing clothes and diapers, and cleaning the individual, his/her clothes and the work area when accidents or spills occur. Report any unusual behavior, incidents, etc. (ex. Seizures) to the administrative office immediately and completes an incident report form. Supervise medication for any individual on the work crew who takes prescription medication during the workday. Report any unauthorized visitors to the administrative office immediately. Substitute for any other trainer as requested. Observes start and ending time and break and lunch time to set an example for workers. Observe and enforce rules prohibiting smoking, eating, or drinking on the vans or in the building. Participate in training to improve training and evaluation skills. Maintain current CPR, 1st Aid, and ACT Certification. Participate on the Review Team, case review Committee, and the Safety committee. Any other assigned duties.

Work Schedule: Monday – Friday 8:30am – 3:30pm.

Salary: $9.00 per hour.
